TO QUALIFY: Free eye exams and glasses will be provided ONLY to children, ages 5 “ 12, who need them based on screening results and qualify based on certain income guidelines. Students who bring vision referrals from school nurses will be given priority on a first-come, first-serve basis. To move quickly through the line, have your school nurse fill out the bottom portion of this form and bring it to the event or bring a copy of the vision referral letter you received from your school.
